Prof Terrance Chua, a clinical cardiologist with an interest in cardiac imaging, has a compelling vision to transform patient care delivery through innovative IT solutions at National Health Centre Singapore (NHCS). As a result of his leadership and relentless push for digital adoption within NHCS, the institution became the only specialist centre to be awarded with the HIMSS EMRAM Stage 6 Certification. This is a huge step forward for NHCS in realising its vision as a Digital Heart Hospital. Prof Chua has developed effective templates with a key focus on seamless patient care, and personally conducted Clinical Documentation training sessions to share good practices. He also provided strong management support to ensure the successful implementation of Computerised Physician Order Entry (CPOE) in NHCS wards.

Prof Chua’s dedication and commitment in leveraging IT as an enabler has had great impact on transforming care delivery in NHCS, exemplified through the implementation of the following:
Clinical Documentation (ClinDoc) in NHCS’ outpatient and inpatient wards, where NHCS achieved one of the highest adoption rates – averaging over 98% – in SingHealth for Outpatient Consult Notes, resulting from Prof Chua’s strong endorsement and IHIS’ support of ClinDoc. In Mar 2017, NHCS successfully extended ClinDoc to inpatient wards;

Front Loading Project (in collaboration with SingHealth Polyclinics and IHiS), where Prof Chua was the Co-chair of the Cardiology Working Group that developed referral protocols between polyclinics and Specialist Outpatient Clinics (SOCs). This has reduced the waiting times for patients referred by SingHealth Polyclinics, the number of trips to NHCS for consultation sessions, as well as discharge rates after the first consultation. The service started with 9 polyclinics in Jan 2015 has been extended to cover all 19 polyclinics.

International Consortium for Health Outcomes Measurement (ICHOM) Online Dashboard, which integrates multiple clinical data sources for value driven outcomes. Prof Chua provided the necessary thought leadership to kick-start the project at the Acute Coronary Syndrome Steering Committee, and subsequently provided crucial support to ensure the success of the project. Its successful implementation has facilitated adherence to the ICHOM reporting standards, including predictive analytics and scorecards, which was adopted by MOH to improve Acute Myocardial Infarction (AMI) outcomes.

Prof Chua is known for his dedication to patient care, teaching and research. A firm believer in leveraging IT to transform and improve the delivery of outpatient and inpatient care, enhance operational efficiency and improve step-down care experience, he is also a key driver in transforming SingHealth’s medical services to encompass seamless multi-disciplinary care.

He holds several key external appointments including the Singapore Heart Foundation, Review Committee for Cardiology and Cardiothoracic Surgery Services, Ministry of Health, and the SAF Cardiology Specialist Advisory Board. He has received several commendations including the National Day Awards’ Public Service Medal (2015), and the National Medical Excellence Awards’ National Outstanding Clinician (2013).
